FOR OUR CURRENT PATIENTS:

Welcome to Online Patient Services! This is a tool for you to use to check out your child's medical information from the comfort of your home or office anytime - day or night. We still ask that you call the office for any issue that needs urgent attention or a same day response. The following is a list of task you can currently do with our online patient service:

• View current prescriptions and request refills
• Lookup last well visit, including date, height and weight
• Examine vaccination and allergy records
• Request appointments for well visits, referrals, school physicals and camp forms
• Unfortunately state forms (ex: 3231 and 3300) CANNOT be printed off due to the requirement of a doctor signature.

All access to Electronic Medical Records is provided on secure pages that are controlled through passwords known only to the parent and appropriate staff. To protect the privacy of your records, from this point forward, all data communication between your computer and our server is encrypted. You can verify the encryption status by looking for a lock icon in the status area of your web browser.

Please login with the primary phone number we have on file and the insurance ID of any patient in your family. By logging in, you attest that you have a legal right to access medical information about the patients associated with the phone number and the insurance ID you provide. Once you have logged in you have the option of changing your login name and your password. If you believe the security of your account has been breached, please call our office during normal business hours.

PARENTS, please note that if your child is a patient and has a chart you are already registered and DO NOT need to register again to create a log-in. If you re-register you are creating a duplicate chart for your child. Please call the office at (404) 303-1314 if you have any questions or concerns.
