accepting new pediatric patients dunwoody gaTo help make your time with us more efficient, this page contains links to important forms that we require all new patients to submit prior to your initial appointment, as well as additional forms and resources that you may need as your family grows. Below are forms from our practice that we ask all new patients to fill out; please print the requested PDFs to be completed by hand. All forms can be faxed to our office (404) 303-1399 and if you cannot find what you are looking for, please feel free to call our main office at (404) 303-1314.

PLEASE NOTE ALL PATIENTS MUST follow these steps to register your child online.

Step 1 – Print out the following new patient paperwork packet and bring with you to your first visit to Children’s Wellness Center prior to setting up a new account within our patient portal.

Step 2 – Access/Register within our Patient Portal
 Now that you have completed your patient paperwork and have been seen by one of our providers, you may access our secure patient portal. In order to access our patient portal, you will need the temporary user name and password that was given to you at the time of your first visit with us. Once you enter this temporary information, you will be given the opportunity to change the user name and password for privacy purposes. This will complete your online registration which will then give you access to your child’s chart immunization records, growth charts, medicines and other important information.

Step 3 – → Go to CHADIS (Child Health & Development Interactive System): CHADIS is our web-based screening, diagnostic and management system that helps your Pediatrician review the health and development of your child.  Using CHADIS before the visit will help us take better care of your child. Once you are registered with CHADIS you will be able to access pre-visit forms, online questionnaires, informational handouts and community resources.
