Children’s Wellness Center is using a new check in process. Pre-visit questionnaires go out approximately 72 hours ahead of your appointment via text message or email. We ask that you fill them out as soon as possible. Please do not wait until you arrive at our office as this will slow down your check in and might delay your appointment time. If you need help completing the forms, please arrive 15 minutes earlier than your appointment time so we may assist you.
